I have checked the changes between version 7 and version 9, and 
also between version 8 and version 9.  I am satisfied that all the 
changes make sense.  I have not checked the changes between the 
previously published draft 2 and the latest version 9, because 
there are too many changes, and they were checked earlier while we 
were producing versions 5 and 7.

I have also checked that the changes requested (relative to 
version 8) have been applied to version 9.

I think it's ready for final touch-up.  I can think of the following
things to do or check:

* remove the blank page that appears as page 1.

* check for bad page breaks, such as where a heading is at the bollom of
a page.

* update the table of contents.
